Standard Care Coordination

Beginning with 1/1.2027 new business and upon renewal, Standard Care Coordination (SCC) will be available for UnitedHealthcare, Oxford, and Surest Level Funded (Small Business & 51+/Key Accounts).

SCC delivers integrated care management designed to educate and empower plan participants to close gaps in care — helping improve health outcomes and lower costs. Support is delivered by a dedicated nurse case manager who answers questions, provides education and resources, stays a consistent point of contact for the duration of the care event, and collaborates with the participant’s primary care provider.

One Pass Select Added to the UHC Store

Level funded plan members will now be able to access the One Pass Select program even if it's not embedded in their plan. They can simply go to the UHC Store!

One Pass Select provides a simple, flexible, and affordable way for employers to elevate their employee wellness offering. The subscription-based fitness and well-being program was developed to help support a healthier lifestyle for members.

The UHC Store is a consumer-centric, digital shopping experience available exclusively to UHC members via the UHC app and myuhc.com. It focuses on health and wellness solutions including everyday health, family wellness, fitness & nutrition, health coordination and more!
