For effective dates October 1, 2026 to March 1, 2027

What is required:

• Employer application
• Enrollment application(s) or Census Enrollment
• Quote
• EFT form
• Prior invoice (without prior Health Net bill, standard underwriting guidelines apply)

Health Net Welcome Home Promo
For existing Anthem groups written alongside Health Net
Anthem will allow members to come on to an Anthem plan as a special enrollment effective:

• October 1, 2026
• November 1, 2026
• December 1, 2026
• January 1, 2027
• February 1, 2027
• March 1, 2027
• During the group's Anthem renewal month

Health Net Welcome Back Promo
Anthem will allow groups to reinstate within 60 days of leaving Anthem.
Group must have left Anthem in good standing
Group will be required to pay back premium
Example: 

• Group cancelled 8/1 to go to Health Net
• Group can reinstate back to 8/1
• Group must pay 8/1, 9/1, and 10/1 all at once
